The Missouri Department of Health and Senior Services (DHSS), along with local public health agencies (LPHA), are working to bring COVID-19 vaccination events to businesses and organizations across Missouri.
Simply complete this form to provide the information they need to connect you to your Regional Implementation Team (RIT), LPHA or other source of local assistance.
The Missouri National Guard has been instrumental in helping the state’s local health departments get Missourians vaccinated. Their aid will end in the very near future, so any employer wanting a clinic at their location should complete the form as soon as possible.
